This notice explains what this website collects and, just as importantly, what it deliberately does not collect.
What this website collects
The appointment request form collects your name, contact details, preferred appointment type, optional destination and travel dates, and preferred appointment times. It does not collect symptoms, diagnoses, vaccine history, health card numbers or any other medical information.
What we never put in URLs or analytics
Form field values are never added to page URLs and never sent to analytics or advertising tools. Analytics events record only that an action happened — for example that a booking button was clicked — without any personal or health information.
Analytics and advertising
The site is prepared for Google Analytics, Google Tag Manager, Google Ads and Meta Pixel through configurable IDs. Where these are enabled by the clinic, they receive event names only. Marketing parameters such as UTM tags are kept separate from appointment request content.
How appointment requests are handled
Requests are transmitted to the clinic through an approved secure channel and reviewed by clinic staff, who confirm details with you directly. A submitted request is not a confirmed appointment.
Your choices
You can request an appointment by telephone instead of using the form. To ask about information the clinic holds about you, contact the clinic directly.
